چکیده

Bacterial infections and sepsis are major problems in critically ill patients. Timely diagnosis and therapy reduce morbidity and mortality. Many studies have included the investigation of various biomarkers whose elevated concentrations can indicate sepsis; among them, PCT proved to be most useful. PCT is synthesized in the thyroid gland as a prohormone of calcitonin. In healthy individuals the PCT concentration is <0.1 ng/mL. The advantage of the PCT is a high negative predictive value for the exclusion of sepsis, with the cut-off value of 0.5 ng/ml. A concentration between 2 and 10 ng/ml indicates strong sepsis, whereas a value ≥10 ng/ml is associated with septic shock. In addition to the diagnosis of sepsis, the measurement of PCT concentration is useful for the introduction and monitoring of antibiotic therapy, which is performed according to an algorithm based on the cutoff value for PCT. Immunoassays are used to measure PCT concentrations in serum or plasma. It is possible to determine the concentration in whole blood by using point-of-care testing. In pathological conditions that are not associated with sepsis, PCT is useful as a prognostic indicator of disease complications. Some studies suggest that PCT is a potential early indicator of acute coronary syndrome.

In these situations, after the elevation of procalcitonin values, a rapid decline is observed in the follow-up measurements. 3 On the other hand, procalcitonin can have false negative or lower values in early course and localized site of an infection. 3 In this context, rep...

Using procalcitonin to guide antimicrobial duration in sepsis: asking the same questions will not bring different answers

Severe sepsis is a major healthcare problem and the early initiation of antimicrobials is one of the few measures associated with improved outcomes. However, antibiotic overuse is an increasing problem in critical care. Of several potential biomarkers for antibiotic stewardship, procalcitonin represents the most widely studied and validated. In this commentary we address the current literature ...

The Evaluation of Serum Procalcitonin Levels in Neonatal Infections

Background Procalcitonin is known as one of the bacteremia and sepsis markers such as cytokines, interleukin and reactive proteins. This study was conducted to determine the procalcitonin levels in neonatal sepsis. Materials and Methods In a cross-sectional study, the serum procalcitonin levels in 50 term newborns with suspected sepsis was compared with 50 healthy newborns in Ghaem Hospital, Ma...

Procalcitonin: A Reliable Marker for the Diagnosis of Neonatal Sepsis

Objective(s) In the last few years, serum procalcitonin has been proposed as an early marker of infections in neonates, with varying results. In this study, we aimed to investigate the value of procalcitonin, and C- reactive protein in establishing the diagnosis of neonatal sepsis. Materials and Methods Blood samples were collected at admission from 69 neonates with suspected infection (admi...

Use of procalcitonin for the detection of sepsis in the critically ill burn patient: a systematic review of the literature.

The purpose of this systematic review was to assess the evidence for use of routine procalcitonin testing to diagnose the presence of sepsis in the burn patient. The electronic databases MEDLINE, Cochrane, CINAHL, ProQuest, and SCOPUS were searched for relevant studies using the MeSH terms burn, infection, procalcitonin, and meta-analysis. The focus of the review was the adult burn population, ...

C-reactive protein and procalcitonin in the assessment of infection response to antibiotic therapy

With these limitations, researchers and clinicians look at the inflammatory cascade for a potential objective serum marker of infection. By definition a serum marker should be absent if the patient is not infected, appear concomitantly and ideally precede the clinical manifestations of infection, disappear with successful therapy or remain elevated if infection is refractory to treatment. Seria...
